29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. Location: 38 Columbus Street, Seymour, CT 06483 Each connector working 23 feet above ground level was not protected from falling with a complete personal fall arrest system. On or about 19 April 2016, an employee upon completing connecting work on a new steel building structure fell 23 feet to the ground while walking on a steel girder en-route to a ladder.
