29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   On or about April 19, 2016, at the above addressed site, employees were expose to fall hazard of approximately 12 feet 6 inches when performing carpentry work on the roof of a building while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about April 19, 2016, at the above addressed site, employees were exposed to a fall hazard when using an extension  ladder to gain access to a roof approximately 12 feet 6 inches in height while the ladder did not extend at least 3 feet past the upper landing surface.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  On or about April 19, 2016, at the above addressed site, employees were exposed to a fall hazard when using an extension ladder to gain access to a roof approximately 12 feet 6 inches that set up on an active construction site and was not secured to prevent accidental displacement.
