29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and the knowledge and skill required for the safe application, usage and removal of the energy controls was acquired by the employees:     An employee working at Danville Brass and Aluminum, Inc. performed service and maintenance activities on the muller and the employer had not developed an adequate energy control program which included training to ensure the purpose and function of the program was understood by the employee.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):     On or about April 7, 2016, an employee working at Danville Brass and Aluminum, Inc. was exposed to the unexpected energization of the muller while conducting servicing and maintenance activities such as replacing the bearing and all energy isolating devices that were needed to control the energy to the muller were not located so as to isolate from its energy source.
