On 04/18/16, at the site of 69-14 Eliot Ave. Middle Village, NY Employees working at ground level where roofing work was being performed over head were exposed to struck-by hazards. Protective helmets were not provided to the employees working on the ground. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) on or about 04/18/16, at 69-14 Eliot Ave. Middle Village, NY Employees were using a portable ladder to gain access to a roof. The ladder side rails extended less than one foot above the upper landing surface was secured, but a grasping device was not provided to assist employees while mounting and dismounting the ladder. The employees were exposed to the hazard of falling approximately 22 ft. to the ground below. b) on or about 04/18/16, at 69-14 Eliot Ave. Middle Village, NY Employee was using a portable ladder to gain access to a lower roof. The ladder side rails extended less than two foot above the upper landing surface, was not secured and did not have a grasping device to assist employees while mounting and dismounting the ladder. The employee was exposed to the hazard of falling approximately 8 ft. to the ground below. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
