29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone [i.e. without the warning line system]:       a) Job site, 320 East North Avenue, Pittsburgh, PA: On or about April 19, 2016, while sweeping roof section "C" (east end of roof), the employer did not ensure that appropriate fall protection systems/devices were used where falls hazards of up to approximately 14 stories existed in that an employee worked in between the flag lines and the edge of the roof without another form of fall protection, such as an appropriate safety monitor system, in use.     b) Job site, 320 East North Avenue, Pittsburgh, PA: On or about April 19, 2016, while rolling and gluing new roof materials down on roof section "C" (east end of roof),  the employer did not ensure that appropriate fall protection systems/devices were used where falls hazards of up to approximately 14 stories existed in that employees worked in between the flag lines and the edge of the roof without another form of fall protection, such as an appropriate safety monitor system, in use.     c) Job site, 320 East North Avenue, Pittsburgh, PA: On or about April 19, 2016, during roofing activities on roof section "A" (west end of roof), the employer did not ensure that fall protection systems/devices were used where falls hazards of up to approximately 14 stories existed. The employer had removed flag lines that were set up for fall protection and did not ensure that another form of fall protection was provided or used while walking/working in this area.       Stringert, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 315561191, citation number 1, item number 1, and was affirmed as a final order on September 8, 2011, with respect to a workplace located at 3850 Washington Road, McMurray, PA.
