29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) Dart Container Corporation (D) Line Bagger - The employer did not effectively guard the point of operation on the D line baggers machine head that heats and perforates the plastic film bagger where employees place heat tape, thereby exposing employees to injuries, on or about April 19, 2016. No abatement certification or documentation required.
