Progressive Tool & Die, Inc.- On or about April 20, 2016, and times prior thereto: Employees worked with or around hazardous chemicals including, but not limited to, denatured alcohol, oils, coolants and lubricants during production operations. The employer had not developed and implemented a written Hazard Communication Program.

Progressive Tool & Die, Inc.- On or about April 20, 2016, and times prior thereto: Safety data sheets for materials including, but not limited to, MWI DC-100 Fluid, Denatured alcohol, Way Lubricant 32 oil, Velocite Oil no. 10, and Tri-Cool Synthetic Based Premium Metal Working Coolant were not maintained at the worksite nor was there a method/procedure in place to ensure SDSs could be provided to an employee requesting access to the document.
