(b) On or about April 20, 2016 and at time prior to that date, for the employees exposed to amputation hazards while operating a HYD-MECH band saw, model S-20P, Serial No. 71198190 with the guard removed in the maintenance area at 1400 North 14th Street, Oakes, North Dakota. (c) On or about April 20, 2016 and at time prior to that date, for the employees exposed to struck-by and/or caught-in hazards while operating a Solberga, Model No. SE1235, Seral No. 976 drill press without a chuck/chip guard in the maintenance area at 1400 North 14th Street, Oakes, North Dakota.
