29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about April 18, 2016, employees working at lower levels were exposed to struck by hazards when employees above them were securing plywood decking to rafters and the employer failed to mandate the use of hard hats. Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about April 18, 2016, employees securing plywood decking to rafters were exposed to a 22.3 foot fall when the employer failed to ensure that fall protection was utilized. No abatement documentation or certification is required for this item.
