29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.       This violation was observed on or about April 20, 2016 at 201 N. Mustang, Mustang, Oklahoma.  Employees working at approximately 14 ft. above ground on a fabricated welded frame sectional scaffold performing EIFS operations were not protected by a complete guardrail system or equivalent means of fall protection.

This violation was observed on or about April 20, 2016 at 201 N. Mustang, Mustang, Oklahoma.  Employees were working at approximately 14 ft. above ground on a fabricated welded frame sectional scaffold without cross bracing between the frames on the inside of the scaffold.

This violation was observed on or about April 20, 2016 at 201 N. Mustang, Mustang, Oklahoma.  The employer did not have a competent person supervise, direct or assist in erecting the fabricated welded frame sectional scaffold without a complete guardrail system, missing cross braces on the inner side, footings not level, missing mid rails on the outer side, exposing employees to fall hazards.

This violation was observed on or about April 20, 2016 at 201 N. Mustang, Mustang, Oklahoma.  Employees working at approximately 14 ft. above the ground on a fabricated welded frame sectional scaffold without a complete guardrail system and without level footings were not adequately trained to recognize the hazards associated with an unsafely constructed scaffold exposing employees to fall hazards.
