29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) On or about April 14, 2016, and at times prior, the employer, Paradox Beer Company, Inc., had not developed and implemented a written hazard communication program to protect employees who are required to use hazardous chemicals including, but not limited to, sodium chlorite, lactic acid, iodine and various types of ammonium chloride. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about April 14, 2016, and at times prior, the employer, Paradox Beer Company, Inc., did not ensure that employees, who are required to use sodium chlorite, a corrosive chemical, were provided access to a flushing station in case of chemical contact. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
