29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: On or about April 21, 2016, the employer failed to ensure that the Baker Bros Inc. P-506 Press #9 (Compression Plastic Press), located in the Plastic Press Room, was protected against accidental contact by employees at the point of operation. The guard that was in place was inadequate in that it did not prevent an employee from reaching under the point of operation.

The employees were using the compressed air nozzles to clean the flash scale of off the Baker Bros Inc. P-506 Press #9 which was located in the Plastic Press Room.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) On or about April 21, 2016, the employer exposed employees to electrical hazards, by utilizing a flexible cord as a substitute for permanent wiring which was used to serve power to a table lamp and a scale that located in the Plastic Press Room. The flexible cord ran from Plastic Press #8 over Plastic Press #9 to power the table lamp and scale.
