29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a)  Uteco Diamond 610 "Tennessee" press, central impression frame unit:  On or prior to 4/20/16, design and construction of the expanded metal barrier guard(s), including but not limited to lack of interlock(s) and presence of openings in the expanded metal enclosure, did not prevent operators from reaching into hazardous locations, such as in-running gear and roll nips, during press set-up and operation.      (b) Uteco Diamond 610 "Tennessee" press, during adjustment of PIV drive:  On or prior to 4/20/16, one or more methods of machine guarding was not used to protect employees from being caught by in-running gear and roll nips while adjusting the PIV unit, including checking the chill drum temperature and when using a contact tachometer to measure the speed of rotating parts.    Salerno Packaging Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 313763567, citation number 1, item number 5 and was affirmed as a final order on March 8, 2012, with respect to a workplace located at 14 Gus Lampham Lane, Plattsburgh, NY 12901.
