29 CFR 1910.95(b)(1): When employees were subjected to sound exceeding those listed in Table G-16, feasible administrative or engineering controls were not utilized:  The employer failed to utilize feasible administrative or engineering controls; For at least one (1) press operator, who was exposed to continuous noise at 203% of the permissible daily noise exposure (8-hour time weighted average sound level of 90 dBA) or equivalent sound level of approximately 95.1 dBA during the 322 minute sampling period on April 21, 2016; exposure calculations included a zero increment for the 158 minutes not sampled.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about April 21, 2016, the employer failed to provide employee(s) voluntarily wearing respirators with the information contained in Appendix D of the respirator standard. Employee(s) voluntarily wear tight-fitting half-mask respirators with cartridges.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:  On or about April 21, 2016, the employer failed to implement the elements of a written respiratory protection program, including medical evaluations and training.
