29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means. Location: 203 East 92nd Street, New York, NY. 23rd Floor. Employee was installing current transformers (CTs) rings in the distribution panel without de-energizing the panel or by utilizing other insulating means. An arc flash occurred between the phases when a tool made contact with the energized bus duct. On or about 04/19/2016.
