29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: a. Site- Employees working from the roof were not protected from a fall of approximately 40 feet where two employees had connected their personal fall arrest systems to the same anchor point. The anchor point was not capable of supporting at least 5,000 pounds per an employee.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. Site- An employee working from the roof was not protected from free falling more than six feet where the employee was connected to a lifeline without the use of a rope grab and had and excessive amount of slack in the lifeline when working at the roof edge.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a. Site- Employees installing metal flashing on the roof with pneumatic nail guns were not protected from eye injuries where eye protection was not worn. Mac Construction & Remodeling, Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 909488, citation number 1, item number 2 and was affirmed as a final order on 10/22/2013, with respect to a workplace located at 15 Long View Way, Georgetown, MA 01833.
