29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:     The employer is failing to ensure that all employees operating powered industrial trucks at the facility have been evaluated and received adequate training as outlined in this paragraph of the standard.  Employees have not been evaluated/re-trained after they have been operating for 3 years.  By accomplishing these training employees would be able to identify the differences of how the vehicle will react as they travel over these terrains.  The employees are exposed to tipping hazards throughout the facility and its surrounding areas.  This most recently occurred on April 22, 2016 on the jobsite located at 917 S. 17th St., Beatrice, NE.
