29 CFR 1926.502(d)(6)(i): A snaphook not designed for the following connections was engaged directly to webbing, rope or wire rope: a) 5000 Cherry Hill - On or about April 22, 2016 and at times prior thereto, employees were exposed to fall hazards from inadequately using their personal fall arrest systems. In particular, the lifelines were not attached or anchored according to the manufacturer, creating the possibility for roll-out in the snap hooks.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: a) 5000 Cherry Hill - On or about April 22, 2016 and at times prior thereto, employees were exposed to fall hazards from inadequately using their personal fall arrest systems. In particular, the lifelines were not attached or anchored according to the manufacturer, creating the possibility for roll-out in the snap hooks. This inadequacy indicates employees must be trained further in fall protection.
