29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  a)  The employer failed to certify and provide documentation to OSHA verifying that abatement had been completed for the following citations:  29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)  On or about April 20, 2016, the controlling employer did not protect employees of Mike Finch Framing who were working on the ground below framing operations from overhead falling object hazards by ensuring the employees were wearing hard hats.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").  and  29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a)  On or about April 20, 2016, and at times prior, the controlling employer, AK Custom Homes, Inc, did not ensure that Mike Finch Framing (subcontractor) protected their employees, who were required to engage in framing operations on a residential house, were protected from exposure to fall hazards of 12 feet and greater.  Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
