OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. (a) On or about 4/19/2016, at the 35431 Scholder Lane, Pala, California worksite, the employer did not ensure excess rope used in tree maintenance operations was stored or secured in such a way to prevent being caught in brush being fed into the chipper. Employees feeding the brush chipper were exposed to rope entanglement and amputation hazards.
