29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:    a. On April 22, 2016, employees on the 3rd floor were dropping scrap materials to the ground 22 feet below without a chute or barricade, exposing employees on the ground to a struck by hazard.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On April 22, 2016, on the 3rd floor, the perimeter of the floor did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to a 22 feet fall hazard.    b. On April 22, 2016, on the 3rd floor, the two elevator openings did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to a 22 feet fall hazard.    c. On April 22, 2016, on the 2nd floor, the south wall and east wall did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to an 11 feet fall hazard.    d. On April 22, 2016, on the 2nd floor, the two elevator openings did not have a guardrail system and employees were not wearing a harness and lanyard, exposing themselves to a 15 feet fall hazard.
