29 CFR 1926.102(a)(2): Eye and face protection required by this Part did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:   (a)  On April 25, 2016, at the front of the house, the employer did not provide an employee using a Ryobi CSB123 12 Amp 7-1/4-in circular saw with safety glasses that meet the requirements specified in Z 87.1-1968.

29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.  (a) On April 25, 2016, at the covered break area, a 4-way, 120 volt outlet adapter/splitter, intended for home use, was used to supply power to a Ryobi CSB123 12 Amp 7-1/4-in Circular Saw.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   (a) On April 25, 2016, at the front of the house, a 50 foot 12/3 AWG  extension cord with damaged strain relief to the outer insulation and wrapped in electrical tape, was used to supply power to a Ryobi CSB123 12 Amp 7-1/4-in Circular Saw.

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:   (a) On April 25, 2016, at the front of the house, employees used a 50 foot 12/3 AWG extension cord with damage to the outer insulation and wrapped in electrical tape, to supply power to a Ryobi CSB123 12 Amp 7-1/4-in Circular Saw.
