29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems: On the jobsite located at 1314 S. 211th Street, Omaha, NE 68022: Employees were observed installing shingles without fall protection while exposed to fall distances between 18 to 30 feet. Albin Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 555218, citation number 1, item number 1 and was affirmed as a final order on October 9, 2012; with respect to a workplace located at 21021 W Street, Elkhorn, NE 68022.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On the jobsite located at 1314 S. 211th Street, Omaha, NE 68022: Employees were using a 28 foot aluminum extension ladder to access the roof. The ladder did not extend at least 3 past the roof level being accessed, was not secured to prevent displacement and there was not a grasping device to help employees safely access the roof. Albin Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1926.1053(b)(1), which was contained in OSHA inspection number 555218, citation number 1, item number 2 and was affirmed as a final order on October 9, 2012; with respect to a workplace located at 21021 W Street, Elkhorn, NE 68022.
