29 CFR 1910.178(o)(1):  The employer did not ensure that only stable or safely arranged loads were handled:     (a)  Timken Motor and Crane Service at 2015 W. 8th Ave., Denver, CO 80239:  On and before April 20, 2016, the employer did not ensure that only stable or safely arranged loads were handled by powered industrial trucks.  Two employees removed scrap metal from the facility yard, an employee operating a forklift and a helper.  The forklift was used to flip over a 3 feet by 5 feet metal plate that weighed approximately 300 pounds.  The helper sustained a broken leg after being struck by the plate after it was flipped over.  This condition exposed employees to struck-by hazards.
