29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap: On or about April 22, 2016, the employer failed to ensure that flexible electrical cords being used in the facility were used in continuous lengths without splice. A flexible cord with two spliced areas, was being used for the heat gun located in the receiving area of the warehouse at 2377 East Enterprise Parkway, Twinsburg Ohio 44087.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about April 22, 2016, employees were exposed to an overloaded reolocatable power tap that was being used to energize the Panasonic microwave Model NN-35N151W , the Ink Jet Turn table TLP604AG0015, and the I-Mark C-84 EZ touch. The relocatable power tap was only rated for 120 VAC, but had 475 VAC total plugged into it.
