29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a) Building Interior, Second Floor: Employees were exposed to a fall hazard of approximately 15 feet while performing framing work on the second (2) floor without the use of a complete guardrail systems or personal fall arrest systems or other means of fall protection in place, on or about  on 4/26/2016.

29 CFR 1926.1053(b)(15): Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use.   a) Building Interior, First Floor: A Louisville extension ladder with visible signs of damage to the feet was not removed from service, on or about 4/25/2016.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a) Building Interior, First Floor: Employees were exposed to a fall hazard of approximately fifteen (15) feet while using a Louisville extension ladder with the feet showing visible signs of damage, on or about 4/26/2016.
