29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 7 Middlefield Ln.  Dothan, Alabama:  On or about April 21, 2016 and at times prior; the employer allowed his employees to perform roofing activities on a high slope roof without fall protection exposing them to a fall hazard of 13.1 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At Gary Sewell Construction:  On or about April 21, 2016 and at times prior; the employer failed to train his employees on the hazards of falls to include but not limited to the nature of fall hazards in the work area and the correct procedures for erecting, maintaining and disassembling fall protection systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 7 Middlefield Ln (Front of House) Dothan, Alabama:  On or about April 21, 2016 and at times prior; the employer failed to ensure an extension ladder used by employees to gain access to and from the roof had its rails extend at least three feet above the landing exposing his employees to a fall hazard of 13.1 feet to ground below.  a) At 7 Middlefield Ln (Side of House) Dothan, Alabama:  On or about April 21, 2016 and at times prior; the employer failed to ensure an extension ladder used by employees to gain access to and from the roof had its rails extend at least three feet above the landing exposing his employees to a fall hazard of 13.4 feet to ground below.
