29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Production Area: The point of operation on the Niagara 90 ton mechanical press brake, model #1B60-8-10, was not guarded in such a manner as to prevent the operator from having any part of his body in the die area during the operating cycle. An employee suffered an amputation injury while operating the press when his fingers were caught in the die area during the operating cycle, on or about April 22, 2016. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED Pennmark Technologies Corporation was previously cited for a violation of this Occupational Safety and Health Administration standard or its equivalent standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1048919, citation number 1, item number 2 and was affirmed as a final order on 9/15/2015, with respect to a workplace located at Lower Demunds Road, Dallas, PA.
