OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    Drilling rig:  On or about April 21, 2016, and times prior to, employees operated a drilling rig during the drilling of an oil/gas well without the derrick being adequately supported.  This exposed the employees to the hazard of being struck by the falling derrick or falling from the derrick to the ground or drilling floor.  The inadequacies included but were not limited to:    a)  All guyline connection loops were formed by only one wire rope clip.    b)  Some of the wire rope clips were installed backwards.    c)  The southeast racking board guyline had numerous broken wires in the loop and along its lower end.    d)  Both north derrick lines were anchored to the mud pump skid.

Beside the ladder not being caged no alternative fall protection form was provided.  This exposed the employees to the hazard of falling from over 20 feet.
