29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about April 25, 2016, an employee was exposed to multiple fractures and/or an amputation hazard when using a stump grinder while the drive belts and pulleys were not provided with a cover.  PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact were at least 10 feet apart:  On or about April 25, 2016, at the above addressed site, an employee was exposed to an electrical shock hazard and was electrocuted when he came into contact with energized overhead power lines carrying 7,620 volts while trimming palm fronds on a palm tree approximately 8 feet away from those power lines.  PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about April 25, 2016, at the above addressed site, employees were exposed to the hazards associated chemicals including but not limited to Krylon Paint, Sea Foam Motor Treatment, Mag1 Engine Oil and Grumbacher Fix-a-Flat while the employer had not implemented a Hazard Communication Program.  PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
