29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Worksite was located at 6 Old Wood Road, Morris Plains, NJ Employees were doing a roof decking without fall protection when an employee fell off the center roof elevation approximately 25 feet to the ground below. Violation occurred on or about 4/25/2016. b) Worksite was located at 6 Old Wood Road, Morris Plains, NJ Two employees were observed doing a roof decking with fall protection harnesses on that were not connected to anchor points or lifelines exposing them to an approximately 25 feet fall to the ground below. Violation occurred on or about 4/25/2016.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. a) Worksite was located at 6 Old Wood Road, Morris Plains, NJ Employees set up and were using a Werner extension ladder to access the roof of the house with the ladder set up in front of the front porch and directly in the path of the front doors of the house while the residents of the house were inside the residence. Violation occurred on or about 4/25/2016.

29 CFR 1926.1053(b)(17): Fixed ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, or corroded components, were not withdrawn from service until repaired. a) Worksite was located at 6 Old Wood Road, Morris Plains, NJ Employees were accessing the rear roof elevation on the back right side of the house using a Louisville ladder, Model L-2222-20, that had a bent left cleat and a broken lower right leg that was damaged where the cleat had snapped off exposing themselves to fall hazards. Violation occurred on or about 4/25/2016. b) Worksite was located at 6 Old Wood Road, Morris Plains, NJ Employees were accessing the rear roof elevation on the back side of the house using a Louisville ladder that had two bent cleats exposing themselves to fall hazards. Violation occurred on or about 4/25/2016.
