29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    In the facility, the employer failed to test employee that were performing activities that exposed employees to noise hazards.         a)  Transmission Department, employee operating machines, including but not limited to, hoists, hand tools, and large metal pieces of material were exposed to an average noise level of 88.2dBA of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 8 hour sampling period.        b)  Motor Department, employees were exposed to 91.9 dBA noise level of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 85 dBA) during the 8 hour sampling period.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):    In the facility, employees were exposed to hazards associated with use of PPE, chemicals, noise, and machines, without a written certified hazard assessment exposing employees to struck by, noise, inhalation, dermal, and fire hazards.
