OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, exposing employees to a fall hazard. On or about 26 April 2016, and at times prior thereto, two employees were exposed to fall hazards while being raised on a pallet separately by a forklift approximately 4 to 7 feet high to dump trash and cardboard into two dumpsters located within the warehouse yard.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders were not located where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons: On or about 26 April 2016, and at times prior thereto, a Hydrogen cylinder was being stored next to an ice machine, ladder, and walkway within the building without the use of a chain or other means to secure the cylinder, exposing employees in the area to a struck by hazard.

29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface. On or about 26 April 2016, and at times prior thereto, a snap switch within the main hallway was missing the faceplate, exposing employees to an electrical hazard of approximately 120 volts.
