29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually, whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure, and to reflect new or revised employee positions with occupational exposure.   On or about April 27, 2016, the exposure control plan covering employees at the Provost Marshal Office was not reviewed or updated on an annual basis.. The last documented review and update was conducted in October of 2009.

29 CFR 1910.1030(d)(3)(i): When there is occupational exposure, the employer did not provide, at no cost to the employee, appropriate personal protective equipment such as, but not limited to, gloves, gowns, laboratory coats, face shields or masks and eye protection, and mouthpieces, resuscitation bags, pocket masks, or other ventilation devices.   On or about April 27, 2016, and at times there prior to, the employer did not provide employees who respond to emergency events where they may be exposed to bloodborne pathogens, with appropriate personal protective equipment at no cost. Employees had to replace latex free/ latex gloves at their own expense once the pair initially issued was contaminated or used. A ready supply of gloves was not available at the work place.
