29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Lathe Department- On May 4, 2016, the employer did not ensure that employees were adequately protected from rotating parts and flying chips from equipment, such as the Mori Seiki ENGL Lathe. The guard on the equipment was not being used while the equipment was in operation.

29 CFR 1910.179(j)(2)(iii): Inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier of the hook inspected, were not performed to ensure that the equipment is operating in a safe and reliable condition: a) VTL Department- On April 27, 2016, the employer did not ensure that damage or deformation of the hook safety latches on crane equipment for M69 and M73 were identified during the daily and monthly inspections and evaluated to ensure that the equipment is operating in a safe and reliable condition.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) CNC Milling, Lathe, and Grinding Departments- On April 27, 2016, the employer did not ensure that guards or methods of guarding were adequate to protect employees from point of operation hazards, rotating parts, and flying chips from equipment, such as M7 Mori Seiki CNC, M18 CNC, R83 150 CNC, R25 Mitsubishi CNC Grinder. The safety interlocks on equipment were damaged, missing or not activated while the equipment was in use. General Manufacturing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1062853, Citation number 1, Item number 3 and was affirmed as a final order on June 10, 2015, with respect to the workplace located at 1725 N. 33rd Avenue in Melrose Park, Illinois 60160.
