29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.    a)   On April 29, 2016, at the address of 129 Anthony Street, Celina, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  On April 27, 2016, at the address of 129 Anthony Street, Celina, OH: The employer did not ensure employees were protected from falls while performing roofing work.  Employees operating in this condition were exposed up to an 18 foot fall to the ground below.

The employees were exposed to potential flying object hazards while operating the pneumatic fasteners.
