29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:       (a)Puetz Corporation @ 1515 15th St. NE, Watertown, SD 57071: On and prior to April 21, 2016, the controlling employer did not ensure that the warning line system that was being utilized as a fall protection system for subcontractor employees conducting roofing activities on the Watertown Community Center was erected around all sides where fall hazards existed. The warning line system used was not fully erected on the north and south ends of the roof up to the point where the parapet wall reached a height of at least 39 inches. This condition exposed subcontractor employees to a fall hazard of approximately 33 feet.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.    (a)Puetz Corporation @ 1515 15th St. NE, Watertown, SD 57071: On and prior to April 21, 2016, the controlling employer did not ensure that the warning line system was connected to the work area by an access path formed by two warning lines. Subcontractor employees accessed the main roof through a roof hatch that did not have warning lines creating a path to the east roof, where subcontractor employees were engaged in EPDM roof installation on the Watertown Recreation Center. This condition exposed subcontractor employees to a fall hazard of approximately 33 feet.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:       (a)Puetz Corporation @ 1515 15th St. NE, Watertown, SD 57201: On and prior to April 21, 2016, the employer did not ensure that each employee engaged in parapet cap installation on the roof of the Watertown Community Center were trained to recognize the hazards of working at elevation and trained in the use of fall protection.  This condition exposed employees to a fall in excess of 33 feet.       Abatement Note: The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas:       a)      The nature of fall hazards in the work area;   b)      The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used;   c)      The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;   d)      The role of each employee in the safety monitoring system when this system is used;   e)      The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs;   f)       The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and   g)      The role of employees in fall protection plans;   h)      The standards contained in this subpart.       Abatement Note:  Abatement certification is required for this item (see enclosed Abatement-Certification Letter").       Abatement Note: 1926.503(b)(1) provides that the employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.         (a)Puetz Construction @ 1515 15th St., Watertown, SD 57201:  On and prior to April 21, 2016, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that, employees were engaged in parapet cap installation on the pool roof of the Watertown Community Center without a fall protection system. This condition exposed employees to a fall hazard of 33 feet.     Puetz Corporation was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Administration Standard, 29 CFR 1926.501(b)(1), which was contained in OSHA Inspection number 441213, Citation number 1, Item 1 issued on July 3, 2012, and became Final Order on July 7, 2012, with respect to a workplace located at 1314 West Haven Avenue, Mitchell, SD 57301.
