29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the time of the inspection, it was determined that the light curtain installed on the freeze plug press did not prevent an employee from getting into the point of operation. On 4/4/2016 an employee received serious injuries when he was caught in the point of operation as the press cycled.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: At the time of the inspection, it was found that the employer did not have machine specific procedures to isolate hazardous energy sources during maintenance of equipment such as the freeze plug press. On 4/4/2016 an employee was cleaning inside a light curtain on the freeze plug press when the press cycled causing serious injuries.
