29 CFR 1926.405(g)(1)(iii)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors: a) 1800 Arch St, Philadelphia, PA - 25th Floor - The energized flexible cord was running through the wall of the C-box thus exposing the employees to electrical shock hazards. This condition was observed on or about April 18, 2016.

The re-locatable power taps were not used in accordance with its listing and labeling, exposing the employees to a potential shock hazard. This condition was observed on April 18, 2016.
