OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fractures from falling: On or about 26 April 2016, employees were exposed to a fall hazard while required to walk and stand upon free turning rollers to load material onto pallets. Employees are also required to walk across the free turning rollers to gain access to their work areas. This exposed employees to injuries from falls. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

A device such as a panic bar that locks only from the outside is permitted on exit discharge doors. On or about 28 April 2016, employee(s) were not able to open an exit route door in the fiber bay and 2 exit route doors in the warehouse area from the inside at all times without keys, tools, or special knowledge: This exposed employees to a smoke inhalation or burn in case of a fire. 29 CFR 1910.147(d)(6): Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. On or about 28 April 2016, employees did not verify the isolation and deenergization of the Fiber Die cutter 128 prior to gaining access to the machine to perform setup. This exposed employees to a caught in hazard. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
