Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) 2400 Old Lincoln Hwy/Roof - on or about April 28, 2016, employees installed insulation onto a low slope roof, working near unprotected sides and edges at heights greater than 14 feet above grade, without utilizing any means of fall protection.    Fischer Contractors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 482878, citation number 1, item number 1a and was affirmed as a final order on November 30, 2012, with respect to a workplace located at 7600 Suite Rd. in Philadelphia, PA.
