Market St. Bethlehem, PA, Worksite: On or about April 29, 2016, employees used tubular welded frame scaffolding to access a pump jack scaffold, approximately 22 feet above grade, but the pump jack scaffold was in excess of two feet above the frame scaffold and a dedicated means of access was not provided.
