29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): A comprehensive personal protective equipment hazard assessment had not been conducted and documented (see 1910.132(d)(2) for documentation requirements) for all work required, such as, but not limited to, changing propane gas cylinders on forklifts, where insulated and/or impermeable gloves and eye protection was not required. High visibility vests and harnesses were required and worn by applicable warehouse workers, but training on the use and inspection (for harnesses and lanyards) was not documented. Abatement documentation is required for this violation.
