29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 4/28/16, at the site of Orchard Park, NY. Employees, on a steep roof of 10 in 12, were not protected by fall protection system(s). Employees were exposed to fall of 20 feet.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) on or about 4/28/16, at the roof area of the site, Orchard Park, NY. The extension ladder side rails did not extend three feet above upper landing. Employees were exposed to fall of 9 feet.   NO ABATEMENT CERTIFICATION REQUIRED
