29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about April 27, 2016, employees were exposed to fall hazards greater than 6 feet in height to unpaved ground level below while installing decking material on an a rooftop without wearing fall protection nor were there any other measures of fall protection in place.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:  On the roof of a residential building, on or about April 27, 2016, employees were exposed to a fall hazard 13 feet in height to the unpaved ground below, while not being trained on the procedures to recognize fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) On the roof of a residential building, on or about April 27, 2015, employees were exposed to a fall hazard greater than 11 feet in height to unpaved ground level below when using a portable ladder that was not 3 feet above the upper landing surface to gain access to a roof.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On a residential building, on or about April 27, 2016, employees were exposed to a fall hazard greater than 11 feet to the unpaved ground below, while using a ladder that was not 3 feet above the upper landing surface and not being trained on ladder hazards.
