29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:     On or about April 26, 2016  near 834 Wakefield where an employee was exposed to a crushed-by hazard when working in an excavation with water accumulation without the employer having taken adequate precautions to protect the employee through the use of cave-in protection, a shield system, or water removal.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:     On or about April 26, 2016 near 834 Wakefield where employees were exposed to fall hazards when using a damaged ladder to enter and exit an excavation without the employer having inspected and withdrawn the ladder from service.
