29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about April 27, 2016, employees performing residential carpentry operations were exposed to struck by hazards while employees handed wood materials to the second level from the first level.  The employer failed to mandate the use of hard hats.    Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about April 27, 2016, employees performing residential carpentry operations were exposed to a 11.2 foot fall when the employer failed to ensure that fall protection was utilized.   The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 997903, citation number 1, item number 3 and was affirmed as a final order on November 13, 2014, with respect to a workplace located at 316 Park Ave., Saint Charles, Illinois 60174.  The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1021507, citation number 1, item number 1 and was affirmed as a final order on March 3, 2015, with respect to a workplace located at State Run lot 502, Elgin, Illinois 60124.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
