29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections:  On or about April 22, 2016 and in times prior thereto two ladders were secured together by a cargo strap to reach a height of approximately 40 feet.  The ladder was located outside of Building B where the side of the building was being painted.  This exposed painters to a fall from elevation hazard in the event the job made connection failed.
