Violation was observed on or about 4/19/2016. b) Work site was located at 304 Broadway, Bayonne, NJ 07002 Employees were exposed to a fall hazard of approximately 20 feet at Stair Tower 2 on the first floor where a doorway had no guardrails or fall protection available to protect employees against the existing fall hazard to concrete and water. Violation was observed on or about 4/19/2016. c) Work site was located at 304 Broadway, Bayonne, NJ 07002 Employees were exposed to a fall hazard of approximately 12 feet at Stair Tower 1 on the first floor where a doorway had no guardrails or fall protection available to protect employees against the existing fall hazard to concrete. Violation was observed on or about 4/19/2016. d) Work site was located at 304 Broadway, Bayonne, NJ 07002 Employees were exposed to a fall hazard of approximately 17 feet at the Elevator Tower on the first floor where a doorway had a top rail that was secured but an unsecured midrail that was hanging from the top rail by tie wire to protect employees against the existing fall hazard to concrete. Violation was observed on or about 4/19/2016.

From this position, employees were pouring concrete into the construction site. Violation was observed on or about 4/19/2016.
