29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were working from a scaffold platform that was not fully planked on the second elevation exposing them to fall hazards of approximately 14 feet.    Violation was observed on or about 4/2/2016.

Violation was observed on or about 4/2/2016.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were exposed to a fall hazard of approximately 10 feet at Stair Tower 2 on the ground floor where a pallet that was unsecured was used to protect employees against the existing fall hazard to concrete and water.    Violation was observed on or about 4/19/2016.    b) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were exposed to a fall hazard of approximately 20 feet at Stair Tower 2 on the first floor where a doorway had no guardrails or fall protection available to protect employees against the existing fall hazard to concrete and water.    Violation was observed on or about 4/19/2016.    c) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were exposed to a fall hazard of approximately 12 feet at Stair Tower 1 on the first floor where a doorway had no guardrails or fall protection available to protect employees against the existing fall hazard to concrete.    Violation was observed on or about 4/19/2016.    d) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were exposed to a fall hazard of approximately 17 feet at the Elevator Tower on the first floor where a doorway had a top rail that was secured but an unsecured midrail that was hanging from the top rail by tie wire to protect employees against the existing fall hazard to concrete.    Violation was observed on or about 4/19/2016.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.    a) Work site was located at 304 Broadway, Bayonne, NJ 07002    Employees were exposed to a fall hazard of approximately 12 feet from the roof of the adult day care center where they were anchoring their lanyards to balusters of a small metal guardrail designed to keep people working on the utilities of the day care center protected from the leading edge of that roof. From this position, employees were pouring concrete into the construction site.    Violation was observed on or about 4/19/2016.
