WORKSITE: The 24-foot aluminum extension ladder, used by employees to access/egress the roof was setup at an angle such that the horizontal distance from the top support to the foot of the ladder exceeded one-quarter the working length of the ladder.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     WORKSITE: The employees performing residential roofing activities on the 6 in 12 pitch roof were not protected against the hazard of falling between 9 feet 5 inches and 10 feet 5 inches to the ground by the use of an appropriate fall protection system.      R.L. Construction was previously cited for a violation of an equivalent Occupational Safety and Health standard 29 CFR 501(b) (11), which was contained in OSHA Inspection Number 988426, Citation Number 1, Item Number 1, issued on 9/11/2014 and affirmed as a final order on 9/25/2014 with respect to a workplace located at1300 Silas Dean Hwy, Wethersfield, CT.
